Start with what an erection actually is
Mechanically, an erection is a blood flow event. Arousal triggers nerve signals that release nitric oxide into the tissue of the penis. Nitric oxide raises levels of a signalling molecule called cGMP. cGMP tells the smooth muscle lining those blood vessels to relax, the vessels widen, blood flows in faster than it drains out, and the tissue becomes firm.
Working against that is an enzyme called PDE5 (phosphodiesterase type 5), whose job is to break cGMP down. In a healthy response there is enough cGMP to stay ahead of PDE5. In erectile dysfunction, that balance tips.
What PDE5 inhibitors do about it
